Introduction About BUTRUM 2MG INJECTION
BUTRUM 2MG INJECTION contains Butorphanol, which belongs to the group of medicines called Opioid agonist/antagonist analgesics. It is used to manage and relieve moderate to severe pain. It is also used as a preoperative/pre-anaesthetic medication, a supplement to balanced anaesthesia, to provide relief from pain during labor and in the management of severe pain for which alternative management is inadequate.
BUTRUM 2MG INJECTION is not recommended for use in patients with lung problems (such as respiratory depression, acute/severe bronchial asthma) and gastrointestinal obstruction (such as paralytic ileus). Before taking BUTRUM 2MG INJECTION, inform your doctor if you have a dependency on narcotics, suffer from cachexia (a condition marked by extreme weight loss and muscle wasting), seizure disorders or hepatic and/or kidney impairment.
BUTRUM 2MG INJECTION should be used with caution in pregnant and breastfeeding women only if necessary. It is not recommended for use in children and adolescents (aged below 18 years). It should be used with caution in elderly patients (aged above 65 years).
The most common side effects associated with the use of BUTRUM 2MG INJECTION are excessive sleepiness, dizziness, nausea, vomiting, nasal congestion, sleeplessness and/or lethargy. Consult your doctor if any of your symptoms get worse.

BUTRUM 2MG INJECTION works by blocking certain receptors (responsible for pain) in the brain and spinal cord which leads to blockage of pain transmission thus providing relief from moderate to severe acute pain in affected individuals.
How to use BUTRUM 2MG INJECTION
BUTRUM 2MG INJECTION will be given to you only by a doctor or a nurse into the vein as an intravenous injection or into the muscle as an intramuscular injection. Your doctor will decide the correct dose and duration for you depending on your age, body weight and disease condition.
Side Effects Of BUTRUM 2MG INJECTION
Common
- excess sleepiness
- dizziness
- nausea and/or vomiting
- lethargy
- headache
- sensation of heat
- signs of dilation of vessels such as nausea, blurred vision, light-headedness or dizziness
- pounding heartbeat
- anorexia (an eating disorder causing weight or weight loss)
- constipation
- dry mouth
- stomach pain
- anxiety
- confusion
- euphoria (feeling very happy)
- floating feeling
- sleeplessness
- nervousness
- stinging sensation
- tremor
- cough
- shortness of breath
- sweating
- itching
- blurred vision
- ear pain
- ringing sensation in ears
- unpleasant taste
Uncommon
- low blood pressure
- transient loss of consciousness
- abnormal dreams
- agitation
- dysphoria (feeling of being unhappy)
- hallucinations (a false perception of reality)
- hostility
- withdrawal symptoms such as fatigue, anxiety, vomiting, sweating, depression
- rashes/itchy welts
- impaired urination
- swelling due to water retention
- chest pain
- high blood pressure
- depression
- shallow breathing
Rare
- dip in blood pressure associated with loss of consciousness during the first hour of dosing of medicine
- severe high blood pressure
- signs of serotonin syndrome (life-threatening drug reaction of serotonin) such as agitation, confusion, dilated pupils
- severe breathlessness in infants
Stop receiving BUTRUM 2MG INJECTION and contact your doctor if you experience any of the following side effects:
- signs of severe hypertension such as anxiety, blurred vision, chest pain
- signs of serotonin syndrome (life-threatening drug reaction of serotonin) such as agitation, confusion, dilated pupils
- stop breastfeeding your baby and consult your doctor immediately if your baby develops increased sleepiness, breathing difficulties and/or limpness
How To Manage Side Effects
Headache
Take rest and relax. Stay hydrated. Do not skip your meals. Consult your doctor if the condition persists or gets worse.
Dizziness
Lie down until the dizziness passes and try to get up slowly. Do not bend down suddenly. Get a lot of rest. Walk slowly and carefully. Take plenty of fluids to stay hydrated. Avoid drinking coffee, alcohol or smoking. Consult your doctor if the condition persists or gets worse.
Constipation
Try to eat more high-fibre foods such as fresh fruit, vegetables, cereals and drink plenty of water. Try doing exercise regularly (going for a daily walk or run). Consult your doctor if the condition persists or gets worse.
Nausea and vomiting
Try to take BUTRUM 2MG INJECTION with or just after a meal or a snack. Stick to simple meals. Avoid eating rich or spicy foods. Consult your doctor if the symptom gets worse.
Warning & Precautions

Pregnancy
BUTRUM 2MG INJECTION should be used with caution during pregnancy only if it is necessary as prolonged use of this medicine might cause physical dependence and neonatal opioid withdrawal syndrome in new-born which can be associated with symptoms such as irritability, abnormal sleep pattern, high-pitched crying and tremors. Consult your doctor before receiving the medicine.

Breastfeeding
BUTRUM 2MG INJECTION should be used with caution in breastfeeding women only if it is clearly necessary as it can pass through breastfed milk. Stop breastfeeding your baby and consult your doctor immediately if your baby develops increased sleepiness, breathing difficulties and/or limpness after breastfeeding. Consult your doctor for advice before receiving the medicine.

Driving and Using Machines
Do not drive or operate machines after receiving BUTRUM 2MG INJECTION as it can impair the mental and/or physical abilities to drive or operate machinery. Consult your doctor before receiving the medicine.

Alcohol
Avoid consumption of alcohol while receiving BUTRUM 2MG INJECTION as it may cause substance abuse, profound sedation, respiratory depression and/or coma.

Kidney
BUTRUM 2MG INJECTION should be used with caution in patients with renal impairment. Consult your doctor before receiving the medicine.

Liver
BUTRUM 2MG INJECTION should be used with caution in patients with hepatic impairment. Consult your doctor before receiving the medicine.

Allergy
Do not receive BUTRUM 2MG INJECTION if you are allergic to butorphanol tartrate and/or any other ingredients in this medicine.

Lungs
BUTRUM 2MG INJECTION is not recommended for use in patients with lung problems such as significant respiratory depression or acute/severe bronchial asthma (in an unmonitored setting). It should be used with caution in patients with other lung diseases such as chronic obstructive pulmonary disease, substantially decreased respiratory reserve, hypoxia, hypercapnia and/or respiratory impairment. Consult your doctor before receiving the medication.

Heart Disease
BUTRUM 2MG INJECTION should be used with caution in patients with heart diseases such as acute myocardial infarction, ventricular dysfunction and/or coronary insufficiency only if clearly necessary. Consult your doctor before receiving the medicine.
Others
BUTRUM 2MG INJECTION is not recommended for use if you:
- have/suspected of gastrointestinal obstruction such as paralytic ileus
Before receiving BUTRUM 2MG INJECTION, inform your doctor if you:
- are dependent on narcotics
- suffer from cachexia (a condition marked by extreme weight loss and muscle wasting)
- are a debilitated patient
- have a head injury and suffer from impaired consciousness and/or coma
- have seizure disorders
Use in Paediatrics
BUTRUM 2MG INJECTION is not recommended for use in children and adolescents (below 18 years of age). Consult your doctor before receiving the medicine.
Use in Geriatrics
BUTRUM 2MG INJECTION should be used with caution in elderly patients (aged 65 years and above). Consult your doctor before receiving the medicine.
Interactions
A. Drug-Drug Interactions:
Before receiving BUTRUM 2MG INJECTION, inform your doctor, if you are taking any of the following medicine:
- sedatives/hypnotics (medicines used to manage sleep disorders) such as barbiturates Ex. amobarbital
- anxiolytics or tranquilizers such as benzodiazepines (medicines used to manage anxiety) Ex. diazepam
- medicines used to manage depression, such as selective serotonin reuptake inhibitors (Ex. citalopram), serotonin and norepinephrine reuptake inhibitors (Ex. paroxetine), tricyclic antidepressants (Ex. amoxapine)
- medicines that can affect the metabolism of certain drugs in the liver (Ex. erythromycin, theophylline)
- monoamine oxidase (MAO) inhibitors (medicines used to manage psychiatric disorders) Ex. linezolid, intravenous methylene blue
- triptans (medicines used to manage migraine) Ex. sumatriptan
- 5-HT3 receptor antagonists (medicines used to manage nausea and vomiting) Ex. ondansetron
- mirtazapine, trazodone, tramadol (medicines that affect the serotonin neurotransmitter system)
- muscle relaxants (medicines used for muscle relaxation) Ex. cyclobenzaprine, metaxalone
- general anaesthetics (medicines used to minimize pain threshold in the whole body) Ex. propofol
- antipsychotics (medicines used to manage psychotic disorders) Ex. risperidone
- other opioids (medicines used to block opioid receptors) Ex. fentanyl
Overdosage:
BUTRUM 2MG INJECTION will be given to you only by a doctor or nurse in a hospital, so it is unlikely for you to receive an overdose. However, if you experience any unusual symptoms, consult your doctor immediately or visit the nearby hospital. Symptoms may include respiratory depression, sleepiness that may prolong to the state of loss of consciousness/coma, skeletal muscle flaccidity, cold & clammy skin, constricted pupils, swelling in the lungs, low heart rate, low blood pressure, partial/complete airway obstruction, atypical snoring and/or hypoxia.
