Introduction About BRUFEN MR CAPSULE
BRUFEN MR CAPSULE is a combination of Ibuprofen and Tizanidine which belongs to the group of medicines called Nonsteroidal Anti-inflammatory drugs and Skeletal Muscle Relaxants. It is used as a painkiller to manage mild to moderate pain including migraine (one-sided headache), pain and inflammation in arthritic diseases (swelling of joints characterised by pain and joint stiffness), degenerative arthritis conditions (Ex. Osteoarthritis in which the cushioning effect caused by the cartilage at the end of the bones wears down causing pain) and soft tissue injury. It is also used to relieve the stiffness and restriction of muscles.
Ibuprofen is a nonselective COX inhibitor that works by blocking the effects of prostaglandin, a substance produced during injury and inflammation that causes pain, on the transmission of pain signals between the brain and nerve endings; tizanidine is a muscle relaxant that blocks the effects of acetylcholine, the neurotransmitter responsible for muscle contraction.
In patients who have liver, kidney, or heart disease, BRUFEN MR CAPSULE should be used with caution. It is not advised during pregnancy or in nursing mothers; consult your doctor for more information. BRUFEN MR CAPSULE is not suitable for use in children below 6 years of age and should be used with caution in elderly patients. The most common side effects of taking BRUFEN MR CAPSULE are dizziness, nausea, headache, diarrhoea, and dry mouth. Consult your doctor if the symptom worsens.
Uses Of BRUFEN MR CAPSULE
- Manage mild to moderate pain including migraine (one-sided headache), pain and inflammation in arthritic diseases (Ex. rheumatoid arthritis), degenerative arthritis conditions (Ex. osteoarthritis) and soft tissue injury
- Relieves stiffness and restriction of muscles resulting from multiple sclerosis, injury, or diseases of the spinal cord

Side Effects Of BRUFEN MR CAPSULE
Common
- drowsiness, tiredness, dizziness
- low blood pressure
- high blood pressure when stopping this medicine suddenly
- dry mouth
- decrease or increase in heart rate
- diarrhoea, feeling sick, vomiting, wind, constipation
- signs of digestive tract ulcer with or without perforation - bowel inflammation and worsening of inflammation of the colon (colitis) and digestive tract (Crohn’s disease) and complications of diverticula of the large bowel (perforation or fistula)
- microscopic bleeding from the intestine which may result in anaemia
- mouth ulcers and inflammation (ulcerative stomatitis) - headache, sleepiness, vertigo, dizziness, fatigue, agitation, insomnia, and irritability
Uncommon
- inflammation of the stomach lining
- signs of kidney problems including swelling, painful urination, decreased urine output
- runny nose, difficulty in breathing (bronchospasm)
Rare
- depression, confusion, hallucinations, sleeping difficulties
- signs of lupus erythematosus syndrome such as butterfly-shaped rashes on the face, joint pain, and stiffness
- signs of increased uric acid levels such as severe joint pain, joint stiffness, difficulty in moving affected joints, redness, and swelling
- signs of kidney problems including swelling, painful urination, decreased urine output
- allergic reactions like itching, rash
- signs of liver damage such as stomach pain, yellowing of skin and eyes
- muscle weakness
Stop taking BRUFEN MR CAPSULE and contact your doctor immediately if you experience any of the following side effects:
- black tarry stools or blood-stained vomit due to stomach ulcer
- swelling of the face, tongue, or throat (larynx) which may cause difficulty in breathing (angioedema), fast heartbeat, low blood pressure or life-threatening shock
- a sudden allergic reaction with shortness of breath, wheezing and drop of blood pressure, hives, itching, purple-coloured rashes with or without fever, asthma attacks
- severe rash with eruptions on the skin especially on the legs, arms, hands and feet, face, and lips [erythema multiforme, Stevens-Johnson´s syndrome or even more severe (Lyell´s syndrome), severe infection with damage of the skin, subcutaneous tissue, and muscle (necrotizing fasciitis)
- heartburn, abdominal pain, indigestion
- visual disturbances, vision loss
- photosensitivity (increased sensitivity to sunlight)
- sudden filling of lungs with water resulting in difficulty breathing, high blood pressure, water retention and weight gain
How To Manage Side Effects

Diarrhea
Drink lots of fluids, such as water or fruit juice, to keep yourself hydrated. Do not consume any medicine on your own without consulting your doctor.

Dizziness Or Drowsiness
When you feel dizzy, lie down or sit down immediately. Lie down in a dark and quiet place with your eyes closed. Vehicle driving should be avoided if you experience frequent dizziness without warning. Discuss it with your doctor if the symptom worsens.

Dry Mouth
A dry mouth can be managed by chewing sugar-free gum or sucking on sugar-free hard candies to stimulate the flow of saliva. Drink water at regular intervals. Do not use mouthwashes that contain alcohol because they can be drying.

Nausea And Vomiting
Take this medicine with meals or just after meals. Stick to simple meals. Try to avoid oil-rich or spicy foods, sugary snacks, and drinks. Avoid drinking too much liquid along with your meals; drink slowly in between meals. If the symptom does not improve, contact your doctor.

Stomach Pain
Try to rest and relax. Try to have smaller and more frequent meals. Keep a heating pad on your stomach. Contact the doctor if the symptom worsens.
Warning & Precautions

Pregnancy
ContraindicatedBRUFEN MR CAPSULE is not recommended in pregnancy and must not be taken during the last 3 months of pregnancy as it may cause serious harm to the baby. Consult your doctor before taking BRUFEN MR CAPSULE.

Breastfeeding
ContraindicatedBRUFEN MR CAPSULE is not recommended for use in breastfeeding women as it may pass through the breast milk to the baby during long-term> management. Consult your doctor for advice before taking BRUFEN MR CAPSULE.

Driving and Using Machines
Use with CautionDo not drive or operate any machines while taking this medicine as it may cause dizziness, tiredness, sleepiness, a feeling of spinning (vertigo), and visual disturbances. Consult your doctor before taking BRUFEN MR CAPSULE.

Alcohol
Consult your doctorAvoid consuming alcohol while taking BRUFEN MR CAPSULE because it can aggravate the side effects.

Kidney

Liver

Heart Disease

Others
BRUFEN MR CAPSULE is not recommended for use if you:
- have stomach or duodenal ulcer
- have dehydration due to diarrhoea, vomiting, or insufficient fluid intake
- have bleeding (including in the brain)
- have the abnormal formation of blood cells related to the condition of unknown origin
Before taking BRUFEN MR CAPSULE, inform your doctor if you:
- have Systemic Lupus Erythematosus (SLE) or mixed type of connective tissue diseases
- have porphyria (inherited disorder of the red blood pigment called haemoglobin)
- have allergies, hay fever, asthma, a chronic swelling of the nasal mucosa, sinuses, adenoids, or chronic obstructive disorders of the respiratory
- have ulcerative colitis (inflammation of the colon with ulcers), Crohn’s disease or other stomach or intestinal diseases
- have problems in the formation of blood cells, bleeding, and clotting disorders
Use in pediatrics:
BRUFEN MR CAPSULE is not recommended for use in children below 6 years of age. Consult your doctor before taking BRUFEN MR CAPSULE.
Use in geriatrics:
BRUFEN MR CAPSULE is used with caution in the elderly as it may increase the risk of bleeding. Before taking BRUFEN MR CAPSULE, consult your doctor.
Interactions
A. Drug-Drug interactions:
- medicine used to manage an abnormal heart rhythm, such as amiodarone, mexiletine or propafenone, digoxin
- cimetidine medicine used for indigestion and digestive ulcers
- antibiotics like fluoroquinolones, enoxacin, pefloxacin or norfloxacin, aminoglycosides (Ex. gentamicin, neomycin)
- rofecoxib (a medicine used as a painkiller)
- contraceptive pills
- ticlopidine (a medicine used to manage blood clots)
- medicine to manage high blood pressure like nifedipine, amlodipine, ace-inhibitors such as captopril, angiotensin-ii receptor antagonists such as losartan, beta blockers like atenolol, propranolol and diuretics (Ex. furosemide, bumetanide)
- sedatives (used as sleeping pills or medicines for anxiety) ex. temazepam
- other NSAIDs including cox-2 inhibitors like nimesulide, celecoxib,
- anticoagulants such as warfarin or heparin, clopidogrel and ticlopidine (a medicine used for managing the clotting of blood)
- methotrexate (a medicine used to manage cancer and auto-immune diseases)
- phenytoin (used in the management of epileptic seizures)
- lithium (a medicine used to manage depression and mania)
- cholestyramine (a medicine used to manage high cholesterol)
- selective serotonin reuptake inhibitors like paroxetine, sertraline, and citalopram (used to manage depression)
- cyclosporine, tacrolimus (medicines used for immunosuppression after organ transplant)
- zidovudine or ritonavir (medicines used to manage patients with HIV)
- probenecid or sulfinpyrazone (medicines for managing gout)
- sulphonylureas like glimepiride (used to manage type 2 diabetes)
- glucocorticoids like prednisolone, and dexamethasone (used against inflammations)
- bisphosphonates like risedronate, and alendronate (medicines used in osteoporosis, Paget’s disease and to reduce high blood calcium levels)
- oxpentifylline or pentoxifylline (used to manage the circulatory disease of the legs or arms)
- baclofen (medicined used as a muscle relaxant)
- gingko biloba herbal medicine
- voriconazole and fluconazole (a medicine used for fungal infections)
Overdosage:
If you or anyone else accidentally takes too much of BRUFEN MR CAPSULE, consult your doctor immediately or visit the nearby hospital as it may increase the risk of side effects. Common side effects are nausea, vomiting, low blood pressure, a slow or abnormal heartbeat, dizziness, small pupils, difficulty breathing, coma, restlessness or sleepiness, stomach pain, and diarrhoea. In addition, headache, gastrointestinal bleeding, blurred vision, ringing in the ears, confusion, and shaky eye movements; exacerbation of asthma in asthmatics; drowsiness; excitement; disorientation; chest pain; palpitations; loss of consciousness; convulsions (mainly in children); blood in the urine; low blood pressure; acute renal failure; liver damage; respiratory depression; cyanosis; a cold body feeling; and difficulty breathing
