INTRODUCTION

TOFADOZ 5 contains Tofacitinib which belongs to a group of medicines called Janus Kinase inhibitors. It is used to treat rheumatoid arthritis, psoriatic arthritis, ulcerative colitis and polyarticular juvenile idiopathic arthritis. Rheumatoid arthritis is a long-term disease that mainly causes pain and swelling of joints.

Tofacitinib is Janus kinase (JAK) inhibitors, which are intracellular enzymes which transmit signals arising from cytokine or growth factor-receptor interactions on the cellular membrane to influence cellular processes of hematopoiesis and immune cell function. It works by inhibiting the JAK-STAT pathway to decrease the inflammatory response.

Before taking TOFADOZ 5, tell your physician if you have liver, kidney or heart problems, are pregnant or breastfeeding. This medicine is not suitable for use in children and adolescents below 18 years of age. The common side effects are headache, nausea, diarrhoea and upper respiratory tract infections.

HOW TOFADOZ 5 WORKS

Tofacitinib works by decreasing cytokines, which is a protein in the body that helps to control the immune system. When a body has an infection, cytokines increase to help fight the infection.

DIRECTIONS FOR USE

Always take TOFADOZ 5 as instructed by your doctor. Swallow whole and do not crush or chew. It can be taken with or without food.

SIDE EFFECTS OF TOFADOZ 5

SERIOUS

  • fever and chills
  • cough
  • skin blisters
  • stomach ache
  • persistent headaches

COMMON

  • lung infection
  • infections of nose, throat or the windpipe
  • urinary bladder infection
  • vomiting
  • diarrhoea

  • indigestion
  • low red blood cell count

  • swelling of the feet and hands, headache, high blood pressure (hypertension)
  • rash
  • shingles
  • influenza, sinusitis

  • increased muscle enzymes in the blood

RARE

  • blood infection (sepsis)
  • disseminated tuberculosis
  • unusual and joint infections

HOW TO MANAGE SIDE EFFECTS

Headache:

Rest and drink plenty of fluids. Apply a wet or cold cloth to forehead and rest in a quiet, comfortable, dark room.

Nausea:

Take normal regular meals and do not eat spicy foods.

Diarrhoea:

Drink plenty of fluids to avoid dehydration. ORS should be used to prevent dehydration.

Vomiting:

Drink clear or ice-cold drinks and eat light, bland foods, avoid sweets.

PREGNANCY

TOFADOZ 5 is not recommended for use in pregnant women unless necessary. Discuss the risks and benefits with your doctor.

BREASTFEEDING

TOFADOZ 5 is not recommended for use in breastfeeding women unless necessary. Discuss the risks and benefits with your doctor.

DRIVING AND USING MACHINES

TOFADOZ 5 has no or limited effect on your ability to drive or handle machines.

KIDNEY

TOFADOZ 5 should be used with caution in patients suffering from kidney diseases. Consult your doctor for advice.       

LIVER

TOFADOZ 5 is not recommended in patients with severe liver problems, including cirrhosis. Consult your doctor for advice.

ALLERGY

Tell your doctor if you are allergic (hypersensitive) to Tofacitinib.

LUNGS

TOFADOZ 5 should be used with caution in patients suffering from chronic lung disease. Consult your doctor for advice.

HEART DISEASE

TOFADOZ 5 should be used with caution in patients suffering from active heart diseases. Consult your doctor for advice.

OTHERS

Tell your doctor before taking this medicine if you have or had:

  • severe infection such as bloodstream infection
  • tuberculosis
  • diabetes
  • HIV/AIDS
  • any type of cancer
  • diverticulitis
  • high blood pressure
  • high cholesterol
  • are planning to get vaccinated

INTERACTIONS

Tell your doctor if you are taking,

  • rifampicin (treats bacterial infections)
  • fluconazole, ketoconazole (treats fungal infections)
  • targeted biologic (antibody) therapies
  • azathioprine, mercaptopurine, ciclosporine and tacrolimus
  • prednisone
