Introduction About AVANAIR 100 TABLET

Avanair 100 Tablet (Avanafil) is a prescription medicine used for the management of erectile dysfunction (ED) in adult men, a condition in which a man has difficulty achieving or maintaining an erection sufficient for satisfactory sexual activity. Avanafil belongs to the PDE-5 inhibitor class and works by increasing blood flow to the penis during sexual stimulation, thereby helping produce and maintain an erection. It is intended for adult men with ED, including men with conditions such as diabetes-related ED, when assessed as appropriate by a healthcare professional.  

Avanair 100 Tablet is generally taken orally, as needed, before anticipated sexual activity, rather than as a regular daily medicine. The recommended timing and dose should be individualized by the prescribing doctor; avanafil has demonstrated a relatively rapid onset, with clinical benefit observed in some men within about 15 minutes. Swallow the tablet with water and do not take more than the prescribed dose or repeat doses more frequently than advised. Food may delay the onset of activity, so patients should follow the product-specific instructions provided by their healthcare professional. Importantly, avanafil does not cause an erection in the absence of sexual stimulation.

Avanafil must not be taken with nitrates or nitric-oxide donors such as nitroglycerin or amyl nitrite, because the combination can cause a potentially dangerous fall in blood pressure. It is also contraindicated with the guanylate-cyclase stimulator riociguat and with certain potent CYP3A4 inhibitors; patients taking alpha-blockers or medicines that lower blood pressure may require particular medical supervision. Patients with significant cardiovascular disease should be assessed before treatment because sexual activity itself may carry cardiovascular risk. Seek urgent medical attention for an erection lasting 4 hours or longer, sudden loss or marked change in vision, or sudden hearing loss. Alcohol can increase the likelihood of dizziness or low blood pressure, and avanafil should not be combined with other erectile-dysfunction medicines unless specifically directed by a doctor.

Avanair is not indicated for use in women, and there are no clinical data establishing its safety in pregnancy or breastfeeding. Common adverse effects reported with avanafil include headache, flushing, nasal congestion/nasopharyngitis and back pain; most reported reactions in clinical trials were mild or moderate. Patients should consult a healthcare professional before use, particularly if they have heart or blood-pressure problems, liver or kidney disease, a history of vision problems, or are taking other prescription or recreational medicines.

During sexual stimulation, nitric oxide promotes the formation of cyclic guanosine monophosphate (cGMP) in penile tissues. By inhibiting PDE-5, avanafil helps prevent the breakdown of cGMP, allowing its effects to persist and supporting increased blood flow to the penis.

Promotes Penile Smooth Muscle Relaxation During Sexual Stimulation

The increased availability of cGMP helps relax the smooth muscle in the penile blood vessels and erectile tissues. How to use AVANAIR 100 TABLET

• Take AVANAIR 100 Tablet exactly as prescribed by your doctor, generally before anticipated sexual activity.
• Swallow the tablet whole with a glass of water; do not crush or chew it unless specifically advised.
• Avanafil should be taken approximately 15–30 minutes before sexual activity, according to your prescribed instructions.
• Sexual stimulation is required for avanafil to work; it does not cause an erection or increase sexual desire on its own.
• Do not take more than one dose in 24 hours and do not exceed the dose recommended by your healthcare professional.

What if I forgot to take AVANAIR 100 TABLET

If you forget to take AVANAIR 100 Tablet before planned sexual activity, there is generally no need to take a missed dose later, as avanafil is usually taken on an as-needed basis rather than as a regular daily medicine. Take it only when required and according to your doctor’s prescribed timing and dose. Do not take an extra tablet to make up for a missed dose or take more than the recommended dose within 24 hours. If you are unsure about when to take your next dose, consult your doctor or pharmacist.

Overdose

Taking more than the prescribed dose of AVANAIR 100 Tablet may increase the risk and severity of side effects such as headache, flushing, dizziness, nasal congestion, indigestion, or changes in blood pressure. Do not take more than the recommended dose or more than one dose within 24 hours. If you accidentally take more AVANAIR 100 Tablet than prescribed, seek medical advice promptly, even if you do not experience symptoms. In case of severe dizziness, fainting, chest pain, vision or hearing changes, or an erection lasting more than 4 hours, seek immediate medical attention.

Allergy

Do not take AVANAIR 100 TABLET if you are allergic to avanafil or any other phosphodiesterase type 5 inhibitor (PDE5) (Ex. Sildenafil).

Heart Disease

AVANAIR 100 TABLET is not recommended for use in patients with heart rate disturbances that may cause cardiac arrest (sudden unexpected loss of heart function, consciousness, and breathing), heart attack, chest pain (angina), variation in blood pressure, or if you have had a heart attack in the past 6 months that will disturb your intercourse. AVANAIR 100 TABLET should be used with caution in patients with heart troubles, which may make it risky for them to have sexual intercourse. Consult your doctor before taking it.

Use In Pediatrics

• Hereditary eye diseases (degenerative retinal disease)
• Loss of vision in one eye because of loss of blood flow to the optic nerve (non-arteritis anterior ischemic optic neuropathy (NAION))

• Manage Stress and Get Adequate Sleep: Practise stress-management techniques and maintain a regular sleep schedule. Psychological stress, anxiety, and inadequate sleep may contribute to sexual difficulties.
• Stay Physically Active and Avoid Prolonged Inactivity: Incorporate movement into your daily routine and avoid sitting for extended periods. Regular activity can help support circulation and general health.

Drug - Drug interaction

1. Nitrates (Nitroglycerin, Isosorbide):

• Interaction: Avanafil may significantly enhance the blood-pressure-lowering effects of nitrates, which can result in severe hypotension, dizziness, fainting, or other cardiovascular complications.
• Management: Concomitant use is contraindicated. Do not take avanafil with nitrates or nitric oxide donors. Seek medical advice if nitrate therapy is required after taking avanafil.

2. Riociguat:

• Interaction: Avanafil may enhance the blood-pressure-lowering effect of riociguat, increasing the risk of symptomatic hypotension.
• Management: Concomitant use is contraindicated. Avanafil should not be used with riociguat or other guanylate cyclase stimulators.

3. Strong CYP3A4 Inhibitors (Ketoconazole, Ritonavir, Clarithromycin):

• Interaction: These medicines can substantially decrease the metabolism of avanafil, resulting in increased avanafil levels in the body and a higher risk of adverse effects.
• Management: Concomitant use should be avoided. Patients taking strong CYP3A4 inhibitors should not use avanafil.

4. Moderate CYP3A4 Inhibitors (Erythromycin, Diltiazem, Fluconazole):

• Interaction: Moderate CYP3A4 inhibitors may increase avanafil exposure and prolong its effects, potentially increasing the risk of adverse reactions.
• Management: A lower maximum dose and longer interval between doses may be required. Follow the doctor’s prescribing instructions regarding dose adjustment.

5. Alpha-Blockers (Doxazosin, Tamsulosin):

• Interaction: Both avanafil and alpha-blockers can lower blood pressure, which may increase the risk of dizziness, light-headedness, or fainting.
• Management: Patients should be stable on alpha-blocker therapy before starting avanafil. Treatment should generally be initiated at the lowest appropriate avanafil dose with monitoring for hypotension.

6. Other Antihypertensive Medicines (Amlodipine, Enalapril):

• Interaction: Avanafil may produce an additional blood-pressure-lowering effect when used with antihypertensive medicines.
• Management: Use with caution and monitor for symptoms of low blood pressure, particularly dizziness or light-headedness.

7. Other PDE-5 Inhibitors or Erectile Dysfunction Medicines:

• Interaction: Combining avanafil with other PDE-5 inhibitors or ED treatments may increase pharmacological effects and the risk of adverse reactions, including hypotension.

Drug - Food interaction

1. Grapefruit / Grapefruit Juice:

• Interaction: Grapefruit and grapefruit juice may inhibit the CYP3A4 enzyme responsible for metabolizing avanafil, potentially increasing avanafil levels in the body and increasing the risk of adverse effects.
• Management: Avoid grapefruit and grapefruit juice around the time of taking AVANAIR 100 Tablet. Follow your doctor’s or pharmacist’s advice regarding dietary restrictions.

2. High-Fat Meals:

• Interaction: A high-fat meal may delay the absorption of avanafil and can therefore delay its onset of action.
