Introduction About ALMET 5MG INJECTION
ALMET 5MG INJECTION contains Metoclopramide which belongs to the group of medicines called Anti-emetics. It is indicated for both adults and children (aged 1 to 18 years) to manage nausea and vomiting which may occur after surgery, chemotherapy or radiotherapy. It can also be used to manage nausea and vomiting which is associated with migraine in adults.
Migraine is a headache which causes severe throbbing pain or pulsing sensation particularly on one side of the head and it is characterized by symptoms such as nausea, vomiting and sensitivity to light.
Your doctor may perform blood tests during management with ALMET 5MG INJECTION to monitor your blood pigment (haemoglobin) levels. Your doctor may immediately or permanently stop your management if there are any abnormalities in the levels of blood pigment.
Before receiving ALMET 5MG INJECTION, inform your doctor if you are having liver, kidney or heart diseases, porphyria (rare blood disorder), brain problems and also if you are using any other medicines that may affect your heartbeat.
ALMET 5MG INJECTION is not recommended for use in patients with epilepsy, parkinson’s disease, bleeding, obstruction or tear in stomach or gut, tumour of adrenal gland (pheochromocytoma) and involuntary muscle spasms or if you are taking a medicine used to manage parkinson’s diseases (levodopa).
ALMET 5MG INJECTION is not recommend for use during the first 3 months of pregnancy and it should be used with caution during pregnancy only if clearly advised by the doctor. It is not recommended for use in breastfeeding women as it may pass through the breast milk.
ALMET 5MG INJECTION is not recommended for use in children (below 1 year of age) and should be cautiously used among elderly patients, hence consult your doctor before receiving it. The most common side effects of receiving ALMET 5MG INJECTION are drowsiness, uncontrollable body movements, diarrhoea, restlessness and feeling weak. Inform your physician immediately if any of the side effect worsens.
Uses Of ALMET 5MG INJECTION
- Prevents nausea and vomiting (that occurs after surgery in adults and children)
- Prevents delayed nausea and vomiting (that occurs after chemotherapy (in children) or caused by radiotherapy (in adults))
- Manage nausea and vomiting caused due to migraine (in adults)
How ALMET 5MG INJECTION Works
ALMET 5MG INJECTION acts and supresses certain centres called dopamine (brain chemical) centres by decreasing the sensitivity of the nerves that transmit signals from gastrointestinal tract to the vomiting centres in the brain. It also relaxes the muscles of stomach and enhances the motility of gut and manages nausea and vomiting.
How to use ALMET 5MG INJECTION
ALMET 5MG INJECTION will be given to you only by a doctor or a nurse into a vein as a slow injection (at least 3 minutes) or can be given as an injection into the muscle (intramuscularly). Your doctor will determine the correct dose and duration for you depending upon your age, body weight and disease condition.
Side Effects Of ALMET 5MG INJECTION
Common
- drowsiness
- uncontrollable movements of the body (such as shaking, tics, twisting movements or muscle stiffness or rigidity)
- rigidity and tremor (similar to parkinson’s disease)
- restlessness
- diarrhoea
- feeling weak
- reduced blood pressure
Uncommon
- elevated levels of hormone called prolactin (which cause milk secretion in men and women who are not breastfeeding)
- irregular periods
- hallucinations
- decreased consciousness
- slow heartbeat
- allergy
- visual problems and involuntary upward deviation of the eyeball
Rare
- confusion
- convulsions (fits) especially in patients with a history of epilepsy (seizures)
Inform your doctor if you experience any of the following side effects after receiving ALMET 5MG INJECTION:
- uncontrollable movements (often head or neck) which may occur in children or young adults
- signs of a brain disorder called neuroleptic malignant syndrome (such as high fever, high blood pressure, convulsions (fits), sweating and production of saliva)
- signs of severe allergic reactions (such as itching
How To Manage Side Effects
Diarrhoea:
Drink lot of fluids such as water or fruit juice to keep yourself hydrated. Avoid eating high-fat or spicy foods. Inform your doctor if the symptom does not improve.
Feeling weak:
Drink plenty of fluids and eat a balanced diet and frequent meals. Follow a regular sleep schedule. Inform your doctor if the symptom does not improve.
Warning & Precautions

Pregnancy
ALMET 5MG INJECTION is not recommended for use during the first 3 months of pregnancy. It should be used with caution during pregnancy only if considered clearly necessary. Consult your doctor before receiving ALMET 5MG INJECTION.

Breastfeeding
ALMET 5MG INJECTION is not recommended for use in breastfeeding women as it may pass through the breast milk and affect your baby. Consult your doctor before receiving ALMET 5MG INJECTION.

Driving and Using Machines
Do not drive or operate any machines if you feel dizzy, drowsy or have uncontrollable twitching, jerking or writhing movements, unusual muscle tone leading to distortion of the body or vision problems after receiving ALMET 5MG INJECTION.

Alcohol
Avoid consumption of alcohol while receiving ALMET 5MG INJECTION because it may increase the sedative effects.

Kidney
ALMET 5MG INJECTION should be used with caution in patients having kidney problems. Your doctor may reduce the dose in patients with moderate or severe kidney problems. Consult with your doctor before receiving ALMET 5MG INJECTION.

Liver
ALMET 5MG INJECTION should be used with caution in patients having liver problems. Your doctor may reduce the dose in patients with severe liver problems. Consult with your doctor before receiving ALMET 5MG INJECTION.

Allergy
Tell your doctor before receiving ALMET 5MG INJECTION if you are allergic to Metoclopramide or any of its constituents.

Heart Disease
ALMET 5MG INJECTION should be used with caution in patients having heart diseases such as history of abnormal heartbeat (QT interval prolongation) or any other heart problems. Consult with your doctor before receiving ALMET 5MG INJECTION.
Others
ALMET 5MG INJECTION is not recommended for use if you:
- have bleeding, obstruction or a tear in your stomach or gut
- have pheochromocytoma (rare tumour of adrenal gland)
- have involuntary muscle spasms (due to other medicines)
- have epilepsy (seizures or fits)
- have parkinson’s disease (brain disorder causing uncontrollable movements of the body)
- had gastrointestinal surgery (such as pyroplasty) in the last 3 to 4 days
- have abnormal levels of blood pigments (methemoglobinemia or NADH cytochrome b5 deficiency
Before receiving ALMET 5MG INJECTION, inform your doctor if you:
- have problem with the levels of salts in the blood (such as potassium, sodium or magnesium)
- have any brain problems
- are suffering from porphyria (rare inherited blood disorder)
Use in paediatrics:
ALMET 5MG INJECTION is not recommended for use in children (below 1 year of age) because of the increased risk of uncontrollable movements. It should be used with caution in children and adolescents aged between (1 to 18 years of age). Your child’s doctor may adjust the dose depending upon child’s age and body weight. Consult your child’s doctor for advice.
Use in geriatrics:
ALMET 5MG INJECTION should be used with caution in elderly patients. Your doctor might adjust your dose depending upon kidney or liver problems and overall health condition. Consult your doctor before receiving ALMET 5MG INJECTION.
Interactions
A. Drug - Drug interactions:
Before receiving ALMET 5MG INJECTION, inform your doctor if you have received or taking any of the following medicines:
- medicines used to manage a brain disorder called parkinson’s disease (Ex. levodopa, carbidopa)
- anticholinergics (medicines used to manage stomach cramps and spasms) Ex. dicyclomine, hyoscyamine
- morphine derivatives (medicine used to manage severe pain) Ex. oxycodone, oxymorphone
- sedative medicines (used to manage sleeping disorders, anxiety, panic attacks) Ex. diazepam, lorazepam
- medicines used to manage mental health problems (Ex. quetiapine, risperidone), medicines used to manage depression (Ex. fluoxetine and paroxetine), neuroleptics (used to manage psychiatric disorders or nausea and vomiting) Ex. chlorpromazine
- medicine used to manage heart failure (Ex. digoxin)
- ciclosporin (used to manage immune problems)
- mivacurium and suxamethonium (medicines used to relax the muscles)
- atovaquone (used to manage pneumonia)
- medicines used to manage pain (Ex. aspirin, paracetamol)
Overdosage:
ALMET 5MG INJECTION will be given to you only by a doctor or nurse in a hospital, and so it is unlikely to receive an overdose. However, inform your doctor if you experience any unusual symptoms such as uncontrollable movements, feeling drowsy, reduced level of consciousness, confusion, hallucinations and cardio-respiratory arrest (loss of function of heart and breathing).

When ALMET 5MG INJECTION is given intravenously, it starts acting within 1 to 3 minutes and if given intramuscularly (into a muscle) it may take up to 10 to 15 minutes. This effect lasts only for 1 to 2 hours. However, this may vary from person to person depending upon the condition. Ask for your doctor’s advice depending upon your specific condition.
Does ALMET 5MG INJECTION increases blood pressure?
ALMET 5MG INJECTION releases catecholamines which is a nerve chemical thought to increase the risk of high blood pressure. It must be used with caution in patients with pre-existing hypertension to manage the risk of any unusual or serious problems. Ask for your doctor’s advice before receiving ALMET 5MG INJECTION.
Is ALMET 5MG INJECTION safe to use during pregnancy?
