    Introduction About TRINICALM FORTE TABLET

    TRINICALM FORTE TABLET is a combination of Benzhexol, Chlorpromazine and Trifluoperazine which belongs to the group of medicines called Anticholinergics and Antipsychotics respectively. It is indicated for the management of schizophrenia. Schizophrenia is a mental disorder characterized by disruptions in thought processes, perceptions, emotional responsiveness and social interactions.

    TRINICALM FORTE TABLET is not recommended for use in patients with liver damage, uncontrolled cardiac decompensation, blood disorders, bone marrow depression, low white blood cells and glaucoma. Contact your doctor before taking. TRINICALM FORTE TABLET should be used with caution in patients with kidney disease, uncontrolled blood pressure, hypotension, arrhythmias (long QT interval), angina pectoris (chest pain), diabetes mellitus and epilepsy. Contact your doctor before taking.

    TRINICALM FORTE TABLET is not recommended for use in pregnancy. Contact your doctor before taking. TRINICALM FORTE TABLET is not recommended for use in breast feeding women as it may pass through the breast milk. Contact your doctor before taking. TRINICALM FORTE TABLET should be used with caution in children and elderly patients. Contact your doctor before taking. Avoid alcohol consumption while taking TRINICALM FORTE TABLET. The most common side effects of this medicine are dry mouth, constipation, blurred vision, mild nausea or nervousness, feeling drowsy or sleepy, weight gain. Contact your physician if any of the symptoms worsen.

    How TRINICALM FORTE TABLET Works

    TRINICALM FORTE TABLET is a combination of Benzhexol, Chlorpromazine and Trifluoperazine where benzhexol works on the brain and helps to improve muscle control and reduces muscle rigidity where chlorpromazine and trifluoperazine works by blocking the action of dopamine, a chemical messenger that affects the mood and thoughts.

    How to use TRINICALM FORTE TABLET

    Take TRINICALM FORTE TABLET as advised by your physician. Swallow the medicine with a glass of water. Do not crush or chew the medicine. Your physician will decide the correct dose and duration for you depending upon your age, body weight and disease condition.

    Side Effects Of TRINICALM FORTE TABLET

    Common

    • Dizziness, light-headedness or feeling faint on stand up or sit up quickly (due to low blood pressure)
    • Change in heart rhythm (QT prolongation in ECG)
    • Trembling or shaking, stiffness of muscles, decreased movements, restlessness increased salivation
    • Involuntary movements of the arms, legs, mouth, tongue and jaw
    • Dry mouth
    • Feeling drowsy or sleepy
    • Weight gain
    • Abnormal breast milk secretion (in men and women)
    • Loss of menstrual periods (in women)
    • Anxiety
    • Hypertonia (increased muscle contractions)

    Rare

    • Breast enlargement in men
    • Inappropriate milk production, altered menstrual cycle (in women)
    • Feeling slowed down or agitation

    Stop taking TRINICALM FORTE TABLET and contact your physician immediately if you experience any of the following side effects:

    • Tardive dyskinesia (repetitive and involuntary movements with grimacing and eye blinking)
    • Swelling, pain and redness in the leg due to formation of blood clots in leg or symptoms such as chest pain and difficulty in breathing
    • Allergic reaction such as rashes, swelling of face, lips, throat or tongue causing difficulty in swallowing or breathing
    • Sweating, irregular blood pressure, pallor, high temperature, rigid muscles, drowsiness, loss of consciousness (symptoms of Neuroleptic Malignant Syndrome)
    • Chest pain (angina) and worsening of pain
    • Sore throat, high fever, feeling very tired, paleness, bruises and nose bleeds or any other infections due to blood problems
    • Fits or seizures
    • Symptoms of paralytic ileus such as abdominal pain and bloating

    How To Manage Side Effects

    Dizziness Or Drowsiness

    Try to rest and relax. Get enough sleep. Avoid driving or operate any machines while you are feeling dizzy. Do not consume alcohol, as it increases dizziness. Contact your doctor if dizziness did not improve.

    Driving and Using Machines

    Do not drive or operate any machines if you feel drowsiness, change in alertness or eye sight problems after taking TRINICALM FORTE TABLET.

    Alcohol

    Avoid alcohol consumption while taking TRINICALM FORTE TABLET. Consult your doctor for advice.

    Kidney

    Use with Caution

    TRINICALM FORTE TABLET should be used with caution in patients with kidney disease. Contact your doctor before taking TRINICALM FORTE TABLET.

    Liver

    Consult your doctor

    TRINICALM FORTE TABLET is not recommended for use in patients with liver damage or jaundice. It should be used with caution in patients with mild to moderate liver disease. Contact your doctor before taking TRINICALM FORTE TABLET.

    Allergy

    Contraindicated

    Do not take TRINICALM FORTE TABLET if you are allergic to Benzhexol, Chlorpromazine and Trifluoperazine or to Phenothiazines, lactose or to other ingredients of this medicine.

    Heart Disease

    TRINICALM FORTE TABLET is not recommended for use in patients with uncontrolled cardiac decompensation and should be used with caution in patients with uncontrolled blood pressure, artificial heart valve, hypotension, coronary insufficiency, heart failure, arrhythmias (long QT interval) and angina pectoris (chest pain). Contact your doctor before taking TRINICALM FORTE TABLET.

    Use In Pediatrics

    TRINICALM FORTE TABLET should be used with caution in children. Your doctor will give the right dose depending on the child’s age, body weight and disease condition. Contact your doctor before taking TRINICALM FORTE TABLET.

    TRINICALM FORTE TABLET should be used with caution in elderly patients. Contact your doctor before taking TRINICALM FORTE TABLET.

    Others

    TRINICALM FORTE TABLET is not recommended for use if you have:

    • Existing blood dyscrasias
    • Bone marrow depression (low number of blood cells)
    • History of low white blood cell counts
    • Glaucoma (increased pressure in the eye)
    • Urinary retention (due to enlargement of prostate gland)
    • History of agranulocytosis
    • Rare hereditary problems of fructose intolerance, galactose intolerance, the Lapp lactase deficiency, sucrase-isomaltase insufficiency or glucose-galactose malabsorption
    • X-ray examination of brain or spinal cord

    TRINICALM FORTE TABLET is not recommended for use if the patient have comatose condition.

    Before taking TRINICALM FORTE TABLET, inform your doctor if you have or had:

    • Parkinson’s disease
    • Hypothyroidism (reduced activity of the thyroid gland)
    • Epilepsy or fits (seizures)
    • History of blood clots
    • Myasthenia gravis (condition where muscles become easily tired and weak leading to difficulty breathing)
    • Pheochromocytoma (high blood pressure due to a tumour near the kidney)
    • Dementia
    • Brain damage
    • Stroke
    • Depression
    • Diabetes mellitus
    • A history of smoking
    • High cholesterol levels

    Interactions

    A. Drug-Drug interactions:

    Before taking TRINICALM FORTE TABLET, inform your doctor if you are taken or taking any of the following medicine:

    • Anticholinergics Ex. dicyclomine (medicine used to manage irritable bowel syndrome)
    • Amphetamine (medicines used to manage Attention deficit hyperactivity disorder)
    • Clonidine, guanethidine, propranolol (medicines used for high blood pressure, heart diseases)
    • Phenobarbital, carbamazepine, diazepam (medicines used to manage fits)
    • Deferoxamine (medicines used to manage iron overload)
    • Prochlorperazine and thioridazine (medicines used to manage schizophrenia, anxiety, nausea and vomiting)
    • Vincristine (medicines used to manage cancer)
    • Quinagolide, cabergoline (medicines used to manage hyperprolactinemia)
    • Citalopram, escitalopram, imipramine (medicines used to manage depression)
    • Medicines used to manage parkinson’s disease (amantadine, bromocriptine, cabergoline, levodopa, lisuride, pergolide, piribedil, ropinirole, selegiline)
    • Sultopride, olanzapine or prochlorperazine (medicines used to manage mental problems)
    • Drugs causing electrolyte imbalance Ex. hydrochlorothiazide
    • Lithium (medicines used to manage mania)
    • Metformin (medicines used to manage diabetes mellitus)
    • Magnesium, aluminium and calcium salts, oxides and hydroxide (medicines used to manage acidity, heart burn, ulcers)
    • Ciprofloxacin, moxifloxacin (medicines used for bacterial infections)
    • Antihistamine Ex. dimenhydrinate (medicines used to manage vertigo)
    • Oral contraceptives (medicines used for managing pregnancy)
    • Disopyramide, amiodarone, quinidine (medicines used to manage arrhythmias)
    • Morphine derivatives Ex. codeine (medicines used to manage severe pain)
    • Butabarbital (medicines used to manage insomnia)
    • Doxazosin, terazosin (medicines used to manage prostate disorders)
    • Anticholinergic medicines used to reduce saliva and lung secretions (Ex. atropine, procyclidine)
    • Warfarin (medicines used to manage blood clots)
    • Metrizamide (medicines used in diagnostic tests)
    • Bethanechol (medicines used to manage urinary retention)
    • Anaesthetics used prior to surgery
    • Adrenaline

    Overdosage:

    If you or anyone else accidentally takes too much of TRINICALM FORTE TABLET, consult your doctor immediately or visit the nearby hospital. Symptoms of overdose are drowsiness, low body temperature, low blood pressure, twisting your limbs, stiffness, shaking, unusual heart beats and coma.

    A: Keep taking TRINICALM FORTE TABLET until your doctor tells you to stop. The doctor will lower your dose gradually. If you stop taking the medicine suddenly you may get withdrawal symptoms including nausea, vomiting, insomnia, tremors, jerky body movements and the inability to control movements of the hands and body after abrupt cessation of antipsychotic drugs. Therefore, a gradual withdrawal is advisable.

    Q: Is TRINICALM FORTE TABLET is safe to use in tardive dyskinesia?

    A: No. TRINICALM FORTE TABLET is not recommended for use in patients with tardive dyskinesia which is an involuntary movement of the tongue, lips and face unless the patient has concomitant Parkinson's disease due to the increased risk of worsening of the patient's condition. Consult your doctor before taking the medicine.
