ESAM is a combination of Amlodipine and Atenolol which belongs to the group of medicine called Calcium channel blockers and Beta-blockers respectively. ESAM is used to treat essential hypertension and angina pectoris. Essential hypertension (also known as primary hypertension) is a medical condition in which the force of the blood exerted against the walls of the artery remains very high, and as a result it may lead to severe damage to the blood vessels of several organs, thus causing heart attack, heart or kidney failure, stroke, or even blindness. Angina pectoris is a type of chest pain that occurs due to reduced or inadequate blood flow or oxygen supply to the heart.  

Both amlodipine and atenolol acts by decreasing the contractile forces of vascular smooth muscle, which results in vasodilatation, thus reducing heart rate and force of contraction. They also reduce chest pain by decreasing myocardial oxygen demand which helps the heart to pump blood more easily.

Before taking ESAM inform your doctor if you have any liver, lung, or kidney disease as a precaution. ESAM is highly not recommended for use in patients having severe heart problems other than hypertension and angina. Consult your doctor for advice before taking ESAM. There is no adequate information about the safety of this medicine on pregnant and breastfeeding women. Consult your doctor before taking.  ESAM is not suitable for use in children aged 6 years or below due to lack of safety data. The most common side effects of taking ESAM are headache, edema, dizziness, tiredness, stomach pain and diarrhea. Consult your doctor if any of the symptom worsens.

ESAM reduces elevated blood pressure and chest pain, where amlodipine works by blocking the influx of calcium ions into the cardiac muscles and atenolol works by inhibiting the beta-1 adrenergic receptors in vascular smooth muscle and the heart, thus leading to a decrease in vascular smooth muscle contractility, increase in smooth muscle relaxation (vasodilatation), a decrease in heart rate and blood pressure and a reduction in myocardial oxygen demand.


Take ESAM as advised by your doctor. Swallow the medicine with a glass of water. Do not crush or chew the medicine. Your doctor will decide the correct dose and duration for you depending upon your age, body weight and disease condition.

  • confusion
  • heart block (can cause dizziness, abnormal heartbeat, tiredness or fainting)
  • nightmares
  • changes in personality (psychoses) or hallucinations.
  • dizziness (when standing up)
  • dry eyes
  • thinning of hair
  • signs of low platelet count (easy bleeding or bruising)
  • jaundice (yellowish of skin or white of eyes)

Stop taking ESAM and contact your doctor immediately if you experience any of the following side effects:

  • signs of allergic reactions (such as raised lumps on your skin, swelling of your face, lips, mouth, tongue, or throat)
  • severe skin reactions such as stevens johnson syndrome, toxic epidermal necrolysis (skin rash, itching, blistering, reddening of skin all over the body, peeling and swelling of the skin, swelling of mucous membrane) 
  • signs of heart block (an abnormal heartbeat, feeling dizzy, tired, or fainting, shortness of breath, chest pain or having swollen ankle)
  • pancreatitis (severe abdominal and back pain with feeling very unwell)


Nausea and vomiting:

Try taking this medicine with or just after meals. Stick to simple meals. Avoid eating rich or spicy food. If the symptom did not improve contact your doctor.


Drink lots of fluids, such as water or fruit juice, to keep yourself hydrated. Avoid taking any medicines without consulting your physician.

Stomach pain:

Try to rest and relax. You can eat and drink slowly or have smaller and frequent meals. Keep a heat pad on your stomach may also help. If the symptom did not improve contact your doctor.


Rest and drink plenty of fluids. Avoid alcohol consumption. You can apply any pain-relieving balm or gel on your forehead to get rid of pain. If the symptom did not improve contact your doctor.


Try to rest and relax. Get enough sleep. Avoid driving or operating any tools or machines while you are feeling dizzy. If the symptom did not improve contact your doctor.



There is no proper information on the use of ESAM during pregnancy and your doctor may prescribe if considered clearly necessary. Consult your doctor before taking ESAM.


ESAM is excreted in breast milk in smaller amounts and should be used with caution only if considered clearly necessary due to increased risk of potential effects in breast-fed infants such as low heart rate (especially in premature infants or infants with impaired kidney function) and low blood sugar levels. Consult your doctor before taking ESAM.


Do not drive or operate any heavy machines if you experience dizziness, sickness, headache, or tiredness. Consult your doctor before taking ESAM.


Alcohol consumption has no significant effects on the activity of this medicine. However, consult your doctor before taking ESAM.


ESAM should be used with caution in patients with severe kidney disease. Consult your doctor for advice before taking ESAM.


ESAM should be used with caution in patients with severe liver disease. Consult your doctor for advice before taking ESAM.


Do not take ESAM if you are allergic to Amlodipine and Atenolol.


ESAM should be used with caution in patients with bronchospastic disease. Consult your doctor for advice before taking ESAM.


ESAM is not recommended for use in patients having severe heart disease (such as cardiogenic shock (heart cannot pump sufficient blood), uncontrolled heart failure, slow heart rate, low blood pressure, and severe peripheral arterial circulatory disturbances). Consult your doctor for advice before taking ESAM.


ESAM is not recommended for you, if you have:

  • untreated phaeochromocytoma
  • metabolic acidosis

Before taking ESAM, inform your doctor if you have:

  • are going to undergo any major surgery
  • have diabetes
  • have thyrotoxicosis (increased thyroid hormones)

Use in pediatrics:

ESAM is not recommended for use in children less than 18 years of age. Consult your doctor for advice.

Use in geriatrics:

ESAM should be used with caution in elderly patients (above 65 years of age). The dose may be initiated with the lowest effective dose and depending upon kidney. liver or heart function. Consult your doctor for advice before taking ESAM.


A. Drug-Drug interactions:

Before taking ESAM inform to your doctor, if you are taking any of these medicines:

  • other antihypertensive medicines (such as reserpine, diltiazem, or clonidine)
  • medicines used to treat abnormal heart rate (e.g., digoxin, disopyramide or amiodarone)
  • digoxin (used to treat heart problem)
  • adrenaline (used to stimulate heart)
  • insulin or oral anti-diabetic agents such as metformin (used to treat diabetes)
  • antifungal medicine (used to treat fungal infections) such as ketoconazole or itraconazole
  • ritonavir, indinavir, or nelfinavir (used to treat HIV infections)
  • rifampicin, erythromycin, clarithromycin (used to treat bacterial infections)
  • non-steroidal anti-inflammatory drugs (NSAIDs) (used to treat pain and inflammation) e.g., indomethacin 
  • medicines to treat nose or sinus congestion or other cold remedies (such as oxymetazoline)
  • simvastatin (used to treat high cholesterol)
  • cyclosporine, tacrolimus, sirolimus, temsirolimus (used to prevent organ rejection after transplant)
  • dantrolene infusion (used to treat body temperature abnormalities)
  • St. John’s Wort (used to treat depression)


If you or anyone else accidentally take too much of ESAM, consult your doctor immediately. Overdosage may cause undesirable effects such as peripheral vasodilation with marked low blood pressure, fast heart rate, lethargy, disorder of respiratory drive, wheezing, sinus pause, slow heart rate, heart failure, bronchospasm, and low blood sugar levels.

Can overdose of ESAM cause death?

If you or anyone accidentally take too much of ESAM, contact your doctor or seek immediate medical attention. Symptoms of overdose may include severely reduced heart rate, blood pressure, a sense of tightness in chest due to cramps in the muscles surrounding the airways or sometimes loss of consciousness and other serious effects. Seeking prompt medical treatment would prevent happening of death in the affected patient.

Can ESAM cause bradycardia?

Yes. ESAM can cause abnormal heart rate (such as bradycardia) in some individuals however not affected by many. Contact your doctor, in case if you experience such symptoms.

Can ESAM cause miscarriage?

There is no proper information on the use of this medicine during pregnancy. However, caution should be exercised when ESAM is administered in women who are already pregnant or trying to become pregnant. Consult your doctor before taking ESAM.

Can ESAM cause bronchospasm?

ESAM has high potential to cause bronchospastic disease among patients who already have an established respiratory disease, and therefore should be used with caution in them. Consult your doctor for advice before taking ESAM.

Is ESAM safe to use in elderly patients?
