INTRODUCTION

TOP NITRO contains Nitroglycerin (glyceryl trinitrate) which belongs to a group of medicines called vasodilators. It is used in the prevention and treatment of chest pain associated with angina pectoris. Angina pectoris is defined as a sudden, sharp, severe pain originating from the apex of heart and radiating to the left shoulder. The pain is severe and crushing, and it is characterized by a feeling of pressure and suffocation, behind breastbone. It can also be used to prevent irritation and intravenous fluid leaking into the surrounding tissues, in patients who are receiving food or drugs directly into a vein.

Nitroglycerin works by relaxing the blood vessels of your heart, and this reduces the strain on the heart by making it easier to pump blood. TOP NITRO is a transdermal patch that is stuck on your skin. The active ingredient (nitroglycerin) is slowly released and absorbed into your skin and blood vessels. The side of the chest is the suitable place to apply the patch.

Along with the treatment, your doctor might ask you to make certain lifestyle changes such as quitting alcohol and smoking, improve your sleep at night, eating a healthy diet and manage weight. The most common side effects of using this patch are nausea, vomiting and headache. Before taking this medicine, inform your doctor if you have anemia, raised pressure in your head, glaucoma, liver, kidney or heart disease apart from angina. If you are pregnant or breast-feeding, consult your doctor for advice before taking this medicine.

HOW TOP NITRO WORKS

Once the transdermal patch is stuck on your skin, the active ingredient (nitroglycerin) is slowly released and absorbed into your skin and blood vessels. Once entering into systemic circulation, nitroglycerin is converted into free radical nitric oxide which activates the enzyme guanylate cyclase. Activation of this enzyme leads to dephosphorylation, which relaxes vascular smooth muscles and blood vessels in your body which increases the amount of blood and oxygen flow to the heart. It reduces cardiac preload and afterload and also reduces spasm of coronary artery, decreasing systemic vascular resistance and blood pressure. This helps in relieving chest pain (angina) and in the management of blood pressure.

DIRECTIONS FOR USE

Always use TOP NITRO as directed by your physician. Wash and dry the area where patch has to be placed. Take the patch and peel of the plastic backing. Place the sticky side of the patch on the chest and hold it for 5 sec. Run your fingers around the edges of the patch to make sure no air or water present. Always, the side of the chest is a suitable place to apply the patch. Your doctor will decide the correct dose for you depending upon your condition. Continue to take this medicine, as long as your doctor tells you to do so, to obtain better results.

HOW TO MANAGE SIDE EFFECTS

Nausea or vomiting:

Try taking this medicine with, or just after, a meal. Stick to simple meals. Do not eat rich or spicy food.

Headache:

Apply hot or cold-water bag on your head. Take rest in a quiet and dark room. Drink tea or coffee.

ALCOHOL

KIDNEY

TOP NITRO should be taken with caution in patients with severe kidney disease. Consult your doctor for advice.

LIVER

TOP NITRO should be taken with caution in patients with severe liver disease. Consult your doctor for advice.

ALLERGY

Do not take this medicine if you are allergic (hypersensitive) to nitroglycerin.

HEART DISEASE

TOP NITRO should be taken with caution in patients with severe heart disease. Consult your doctor for advice.

OTHERS

Do not use this medicine, if you:

  • have headache or increased intracranial pressure
  • are hospitalized due to severe dehydration or loss of blood
  • have severe anemia

INTERACTIONS

Talk to your doctor, if you are taking,

  • medicines used to treat depression (tricyclic antidepressants)
  • medicines used to treat migraine (ergot alkaloids)
  • medicines used to treat erectile dysfunction (e.g. sildenafil, tadalafil or vardenafil)
  • medicines used to treat high blood pressure (propranolol, captopril)
  • other medicine used to treat angina such as long acting nitrates (isosorbide dinitrate, isosorbide mononitrate)
  • non-steroidal anti-inflammatory drugs (used to relieve pain) e.g. aspirin
  • diuretics (used to increase urine output)
  • tranquillizers (used to reduce fear or anxiety) e.g. chlorpromazine, flupentixol, haloperidol, clozapine
