INTRODUCTION

R ZEP 2MG TABLET contains Pimozide which belongs to the group of medicines called Neuroleptics. It is used in the treatment of illnesses affecting the way you think, feel or behave. These illness may make you feel confused, see, hear or feel things that are not there (hallucinations), believe things that are not true (delusions) and feel unusually suspicious (paranoia).

R ZEP 2MG TABLET is not recommended for use in patients who had an irregular heartbeat or unusually slow heartbeat, recent history of heart attack or have heart failure, parkinson’s disease, depression. Consult your doctor before taking it.

R ZEP 2MG TABLET should be used with caution in patients with liver, kidney, heart problems, fits, thyroid problems. Consult your doctor before taking it.

R ZEP 2MG TABLET is not recommended for use in pregnant women, particularly during the first three months of pregnancy, unless, considered by the doctor. R ZEP 2MG TABLET is also not recommended if you are a breastfeeding woman. Consult your doctor before taking it.

R ZEP 2MG TABLET is not recommended for use in children below the age of 12 years. R ZEP 2MG TABLET should be used with caution in elderly patients with dementia. Consult your doctor before taking it.

The most common side effects of R ZEP 2MG TABLET are sweating more than usual, urinating at night, feeling dizzy or feeling sleepy. Consult your doctor if any of these symptom gets worse.

HOW R ZEP 2MG TABLET WORKS

R ZEP 2MG TABLET is a neuroleptic that acts in the brain by binding to certain receptors called dopamine D2 receptors. Thereby, decreases the abnormal excitement in the brain.

DIRECTIONS FOR USE

Take R ZEP 2MG TABLET as advised by your doctor. Swallow the medicine with a glass of water. Do not crush or chew the medicine. Your doctor will decide the correct dose and duration for you depending upon your age, body weight and disease condition.

Do not stop taking this medicine unless your doctor tells you otherwise, you should stop taking R ZEP 2MG TABLET gradually. Stopping treatment suddenly may cause symptoms such as nausea, vomiting, difficulty sleeping, sweating.

SIDE EFFECTS OF R ZEP 2MG TABLET

COMMON

  • sweating more than usual
  • urinating (passing water) at night
  • feeling dizzy, sleepy
  • feeling agitated or restless
  • feeling low or depressed
  • difficulty sleeping
  • feeling tired or lacking in energy
  • loss of appetite
  • dry mouth, vomiting, constipation
  • headache
  • blurred vision
  • very oily skin
  • need to pass water (urine) more often than usual
  • erection problems in men
  • weight gain

UNCOMMON

  • no monthly period in women
  • swollen face

RARE

Stop taking R ZEP 2MG TABLET and consult with your doctor immediately if you experience any of the following symptoms:

  • severe allergic reaction such as rash, hives (urticaria), severe irritation of skin
  • a serious problem called ‘neuroleptic malignant syndrome’ characterised by fast heartbeat, changing blood pressure and sweating followed by fever, faster breathing, muscle stiffness and loss of consciousness and may lead to coma
  • fast heartbeat (tachycardia), abnormal heartbeat (arrhythmia), fluttering of heart
  • jerky movements and problems such as slowness, muscle stiffness or spasm, shaking, trembling or tremors, feeling restless and stiff neck, more saliva than normal, twitching or unusual movements of the tongue, face, mouth, jaw or throat, difficulty speaking or rolling of the eyes
  • low sodium levels in the blood which can cause tiredness and confusion, muscle twitching, fits or coma
  • symptoms of blood clots in the veins especially in the legs (include swelling, pain and redness in the leg), which may travel through blood vessels to the lungs causing chest pain and difficulty in breathing

HOW TO MANAGE SIDE EFFECTS

Headache:

Rest and relax. Drink plenty of fluids such as water or ORS (Oral Rehydrating Solution) and apply a pain-relieving balm on the head if necessary. Contact your doctor if the headache did not improve.

Dizziness:

Try to rest and relax. Get enough sleep. Avoid driving or operating any machines while you are feeling dizzy. Do not drink alcohol, as it can aggravate your dizziness. Contact your doctor if your dizziness did not improve.

Vomiting:

Take R ZEP 2MG TABLET, with or just after a meal. Stick to simple meals. Avoid eating oily or spicy food. Contact your doctor if your vomiting did not improve.

Constipation:

Take fibre-rich foods in your diets such as fresh fruits, vegetables and whole grain cereals and drink plenty of fluids. Exercise more regularly and stay active. Contact your doctor if constipation persists.

WARNING & PRECAUTIONS

PREGNANCY

The safety of R ZEP 2MG TABLET in pregnancy has not been established. Therefore, it is not recommended for use in pregnant women, particularly during the first trimester of pregnancy, unless, considered by the physician.

If R ZEP 2MG TABLET is used in the last trimester (last three months of their pregnancy): shaking, muscle stiffness and/or weakness, sleepiness, agitation, breathing problems and difficulty in feeding can occur in a newborn babies. Consult your doctor before taking R ZEP 2MG TABLET.

BREASTFEEDING

R ZEP 2MG TABLET is not recommended if you are a breast-feeding woman as small amount of medicine may pass into mother’s milk. Consult your doctor before taking R ZEP 2MG TABLET.

DRIVING AND USING MACHINES

Avoid driving or using machine without consulting with your doctor as R ZEP 2MG TABLET may affect your ability to drive or use machines.

ALCOHOL

Consumption of alcohol is not recommended while taking R ZEP 2MG TABLET as it may make you feel drowsy and less alert.

KIDNEY

R ZEP 2MG TABLET should be used with caution in patients with kidney problems. Consult your doctor before taking R ZEP 2MG TABLET.

LIVER

R ZEP 2MG TABLET should be used with caution in patients with liver problems. Consult with your doctor before taking R ZEP 2MG TABLET.

ALLERGY

R ZEP 2MG TABLET should not be used if you are allergic to pimozide or to similar medicines or any of the other ingredients of this medicine. Consult with your doctor before taking R ZEP 2MG TABLET.

HEART DISEASE

R ZEP 2MG TABLET is not recommended for use in patients who had an irregular heartbeat (arrhythmia) or unusually slow heartbeat (bradycardia), QT-prolongation (abnormal heart rhythm), recent history of heart attack or have heart failure.

Before taking R ZEP 2MG TABLET inform your doctor if you have a heart problem or anyone in your close family has died suddenly of heart problems. 

OTHERS

R ZEP 2MG TABLET is not recommended for you, if you have:

  • parkinson’s disease
  • depression
  • less consciousness of things around you or your reactions become slower
  • lower levels of minerals (electrolytes) in your blood

Before taking R ZEP 2MG TABLET, inform your doctor if you have:

  • epilepsy or other problems that can cause fits (convulsions)
  • problems with your thyroid gland
  • non-cancerous tumour of the adrenal gland (phaeochromocytoma)
  • any history of blood clots or if someone else in your family has

Use in pediatrics:

R ZEP 2MG TABLET is not recommended for use in children below the age of 12 years. Check with your doctor before taking R ZEP 2MG TABLET.

Use in Geriatrics:

R ZEP 2MG TABLET should be used with caution in elderly patients with dementia. Consult with your doctor before taking R ZEP 2MG TABLET.

INTERACTIONS

A. Drug-drug interactions:

Before taking R ZEP 2MG TABLET, inform your doctor if you are taking any of the following medicine:

  • ketoconazole, itraconazole, miconazole, fluconazole (medicines used in the treatment of fungal and yeast infections)
  • erythromycin, clarithromycin, azithromycin, troleandomycin (medicines used in the treatment of bacterial infections)
  • indinavir, ritonavir, saquinavir (used in the treatment of HIV infection)
  • quinidine, disopyramide, procainamide, amiodarone, sotalol, bretylium (used in the treatment of irregular heart rhythm)
  • terfenadine (used to treat symptoms caused by allergies)
  • cisapride (used in the treatment of digestive problems)
  • mefloquine, quinine (used in the treatment and prevention of malaria)
  • nefazodone, amitriptyline, maprotiline, sertraline, paroxetine, citalopram, escitalopram (medicines used in the treatment of depression)
  • chlorpromazine, thioridazine, sertindole (medicines used in the treatment of mental illness)
  • sibutramine (used for weight control)
  • tranquillisers (used in the treatment of anxiety and sleeping problems) Ex. diazepam, alprazolam
  • strong pain killers (used for the treatment of severe pain) Ex. tramadol
  • anti-epileptic drugs Ex. phenytoin, carbamazepine (used in the treatment of fits)
  • metoclopramide (used in the treatment of nausea & vomiting)
  • diuretics, calcium channel blocker (used in the treatment of high blood pressure) Ex. hydrochlorothiazide, nicardipine, nifedipine

B. Drug-food interactions:

Do not drink grapefruit juice while taking R ZEP 2MG TABLET.

Over dosage:

If you or anyone else accidentally take too much of R ZEP 2MG TABLET, consult your doctor immediately or visit the nearby hospital. Symptoms of overdose of R ZEP 2MG TABLET are severe extrapyramidal reactions, hypotension or sedation, cardiac arrhythmias, a comatose state with respiratory depression.
