INTRODUCTION

MIMCIPAR 30MG contains Cinacalcet which belongs to a group of medicines called Calcimimetics. MIMCIPAR 30MG is used in adults and children (aged 3 to less than 18 years) in the treatment of secondary hyperparathyroidism with serious kidney disease in whom dialysis is needed to clear blood from waste products. The medicine is also used in the treatment of parathyroid cancer and primary hyperparathyroidism in adult patients. Primary hyperparathyroidism is a condition where one or more parathyroid glands (pea shaped glands) produces more parathyroid hormone (responsible for controlling calcium levels in blood) which results in the elevated levels of calcium in the blood characterized by frequent urination, increased thirst, joint and kidney pain. Secondary hyperparathyroidism is a condition that causes loss of calcium in blood which is characterized by weak bones, joint pain, kidney stones, with increased urination.

Cinacalcet is a calcimimetic which works by reducing the level of parathyroid hormone through extracellular calcium which increases the sensitivity of the calcium sensing receptors to get activated resulting in the inhibition of parathyroid hormone thus decreasing the level of calcium in the blood. Along with the treatment, your doctor might want you to take periodic blood tests to monitor the level of calcium in the blood.

Before taking MIMCIPAR 30MG, inform your doctor if you ever had seizures, intolerant to certain sugars, and have heart, kidney or liver disease. MIMCIPAR 30MG is not recommended for use if you have signs of hypocalcemia (low calcium level) and allergic skin reactions (such as swelling of the face, lips, mouth, tongue or throat). It should be used in pregnant and breast-feeding women only if it is clearly advised by your doctor. MIMCIPAR 30MG is not recommended for use in children and adolescents (below 18 years) with parathyroid cancer (a rare cancerous disease of parathyroid gland) or primary hyperparathyroidism (a condition that causes loss of bone tissue). It is used in children (aged 3 to less than 18 years) with secondary hyperparathyroidism (a condition that causes low calcium levels in blood) which is advised to be taken after calcium levels are monitored by your doctor before taking MIMCIPAR 30MG. The most common side effects of taking MIMCIPAR 30MG are dizziness, nausea and vomiting, muscle pain, weakness, allergic reactions such as rash, headache, and constipation. Consult your doctor if any of these symptoms gets worse.

USES OF MIMCIPAR 30MG

  • Secondary hyperparathyroidism (HPT) in patients with chronic kidney disease (CKD) on dialysis
  • Hypercalcemia in patients with Parathyroid carcinoma and primary hyperparathyroidism (HPT)

HOW MIMCIPAR 30MG WORKS

MIMCIPAR 30MG works by reducing the level of parathyroid hormone through extracellular calcium which increases the sensitivity of the calcium sensing receptors to get activated resulting in the inhibition of parathyroid hormone thus decreasing the level of calcium in the blood.

DIRECTIONS FOR USE

Take MIMCIPAR 30MG as advised by your physician. Swallow MIMCIPAR 30MG with a glass of water and do not chew or crush the medicine. MIMCIPAR 30MG can be taken with or shortly after food. Your doctor will decide the correct dose and duration for you depending upon your age, body weight and disease condition.

SIDE EFFECTS OF MIMCIPAR 30MG

COMMON

  • dizziness
  • nausea and vomiting
  • loss or decrease of appetite
  • muscle pain
  • weakness
  • allergic reactions such as rash 
  • headache
  • seizures
  • low blood pressure
  • signs of upper respiratory infection (such as cough, runny nose, sneezing and throat pain)
  • breathing difficulties
  • cough
  • indigestion
  • diarrhea
  • upper abdomen pain
  • constipation
  • muscle spasms
  • back pain
  • numbness or tingling sensation in hands, legs, or feet

UNCOMMON

  • muscle aches or cramps
  • sudden tightening of the muscles in the hands, feet, face, or throat
  • infection of dialysis access (surgically created blood vessel where during dialysis blood leaves and enters the body)

RARE

Stop taking MIMCIPAR 30MG and contact your doctor if you experience any of the following side effects:                   

  • signs of hypocalcemia (low calcium level) such as spasms, twitches, or cramps in your muscles, or numbness or tingling in your fingers, toes or around your mouth or seizures, confusion or loss of consciousness
  • signs of allergic skin reactions such as swelling of the face, lips, mouth, tongue or throat which may cause difficulty in swallowing or breathing

HOW TO MANAGE SIDE EFFECTS

Dizziness:

Try to rest and relax and get enough sleep. Try to avoid driving or operating any tools or machines while you are feeling dizzy. Limit your consumption of alcohol, as it can aggravate your dizziness. Consult your doctor if the symptom does not improve.

Decreased appetite:

Eat when you feel hungry. Eat smaller meals frequently than usual. Snack only when you're hungry. Eat a nutritious snack which is rich in calories and protein, such as dried fruit and nuts. Consult your doctor if the symptoms get worse.

Nausea and vomiting:

Try to take MIMCIPAR 30MG with or just after a meal or a snack. Stick to simple meals. Avoid eating rich or spicy food. Consult your doctor if the symptom gets worse.

Headache:

If MIMCIPAR 30MG causes headache, then take rest and drink plenty of fluids. Ask your doctor to recommend a painkiller. Paracetamol is safe to be taken with MIMCIPAR 30MG. Headaches should usually go away after the first week of taking MIMCIPAR 30MG. If headaches last more than a week or severe, inform your doctor.

Diarrhea:

Drink lots of fluids, such as water to keep yourself hydrated. Try to avoid taking any medicine on your own to treat diarrhea and consult your doctor if the symptom gets worse.

Indigestion:

Try to reduce consumption of tea, coffee, cola or alcohol and lose weight if you are obese. Avoid smoking and do not eat three to four hours before going to bed.

Constipation:

Try to eat more high-fiber foods such as fresh fruit, vegetables, cereals, and drink plenty of water. Try doing exercise regularly (going for a daily walk or run). If this does not help, inform your doctor for receiving alternate treatments for constipation.

WARNING & PRECAUTIONS

PREGNANCY

MIMCIPAR 30MG should be used during pregnancy only if it is considered clearly necessary. Consult your doctor before taking MIMCIPAR 30MG.

BREASTFEEDING

MIMCIPAR 30MG should be used only if it is clearly necessary in breastfeeding women. Consult your doctor before taking MIMCIPAR 30MG.

DRIVING AND USING MACHINES

MIMCIPAR 30MG may cause dizziness or seizures which could affect your ability to drive or operate machines. If you experience these side effects, do not drive or operate machines. Consult your doctor before taking MIMCIPAR 30MG.

ALCOHOL

Avoid consumption of alcohol while taking MIMCIPAR 30MG. Consult your doctor before taking MIMCIPAR 30MG.

KIDNEY

MIMCIPAR 30MG should be used with caution in patients with kidney disease. Consult your doctor before taking MIMCIPAR 30MG.

LIVER

MIMCIPAR 30MG should be used with caution in patients with moderate and severe liver disease. Consult your doctor for advice before taking MIMCIPAR 30MG.

ALLERGY

MIMCIPAR 30MG is not recommended for use if you are allergic to Cinacalcet or any of the other ingredients of this medicine.

HEART DISEASE

MIMCIPAR 30MG should be used with caution in patients with heart diseases such as heart failure. Consult your doctor before taking MIMCIPAR 30MG.

OTHERS

MIMCIPAR 30MG is not recommended for use if you:

  • have signs of hypocalcemia (low levels of calcium in blood) such as twitching of hands, face, and feet, memory loss and scaly skin.

Before taking MIMCIPAR 30MG, inform your doctor if you:               

  • have or ever had seizures 
  • have intolerance to some sugars

Use in Pediatrics:

MIMCIPAR 30MG is not recommended for use in children and adolescents (below 18 years) with parathyroid cancer (a rare cancerous disease of parathyroid gland) or primary hyperparathyroidism (a condition that causes loss of bone tissue). It is used in children (aged 3 to less than 18 years) with secondary hyperparathyroidism (a condition that causes low calcium levels in blood) which is advised to be taken after calcium levels are monitored by your doctor before taking MIMCIPAR 30MG.

Use in Geriatrics:

MIMCIPAR 30MG is used with caution in elderly patients (aged 65 years or above). Consult your doctor before taking MIMCIPAR 30MG.

INTERACTIONS

A. Drug-Drug Interactions:

Before taking MIMCIPAR 30MG, inform your doctor, if you are taking any of the following medicine:

  • etelcalcetide (medicine used in the treatment of secondary hyperparathyroidism)
  • medicines used to treat skin and fungal infections (Ex. ketoconazole, itraconazole and voriconazole)
  • medicines used to treat bacterial infections (Ex. telithromycin, rifampicin and ciprofloxacin)
  • medicine used to treat HIV infection and AIDS (Ex. ritonavir)
  • medicine used to treat depression (Ex. Fluvoxamine, amitriptyline, desipramine, nortriptyline and clomipramine)
  • medicine used to relieve cough (Ex. dextromethorphan)
  • medicines used to treat changes in heart rate (Ex. flecainide and propafenone)
  • medicine used to treat high blood pressure (Ex. metoprolol)

Overdosage:

If you or anyone else accidentally takes too much of MIMCIPAR 30MG, consult your doctor immediately or visit the nearby hospital. Symptoms of overdose are numbness or tingling around the mouth, muscle aches or cramps and seizures.
