INTRODUCTION

GTN contains Nitroglycerin (glyceryl trinitrate) which belongs to a group of medicines called vasodilators. It is indicated for acute relief of an attack or prophylaxis of angina pectoris due to coronary artery disease. Angina pectoris is defined as a sudden, sharp, severe pain originating from the apex of heart and radiating to the left shoulder. The pain is severe and crushing, and it is characterized by feeling of pressure and suffocation, behind breastbone.

Nitroglycerin works by relaxing the blood vessels of your heart, and this reduces the strain on the heart by making it easier to pump blood. Along with the treatment, your doctor might ask you to make certain lifestyle changes such as quitting alcohol and smoking, improve your sleep at night, eating a healthy diet and manage weight. The spray is meant for use under your tongue and should not be inhaled. It is recommended to spray 1 or 2 puffs under your tongue. The most common side effect of taking this medicine are headache and dizziness. Before taking this medicine, inform your doctor, if you have anemia, raised intracranial pressure, liver, kidney or heart disease.

HOW GTN WORKS

GTN is converted into free radical nitric oxide which activates the enzyme guanylate cyclase. Activation of this enzyme leads to dephosphorylation, which relaxes vascular smooth muscles and blood vessels in your body which increases the amount of blood and oxygen flow to the heart. It reduces cardiac preload and afterload and also reduces spasm of coronary artery, decreasing systemic vascular resistance and blood pressure. This helps in relieving chest pain (angina) and in the management of blood pressure.

DIRECTIONS FOR USE

Always take GTN as directed by your physician. The spray is meant for use under your tongue and should not be inhaled. At the onset of an attack, rest or sit quietly. Hold the bottle upright and place your finger on the button. Open your mouth and fold your tongue, exposing the sublingual region. Place the bottle near your chin. Press the button tightly so that the puff of medicine goes under your tongue. Close your mouth. Do not inhale the medicine, while you are taking the puff of medicine.

It is recommended to spray 1 or 2 puffs under your tongue. If symptoms do not resolve, repeat at 5-minute intervals for a maximum of 3 puffs. If the symptoms still persist, please seek immediate medical attention.

SIDE EFFECTS OF GTN

SERIOUS

  • allergic skin reaction (with or without rash)
  • tongue blistering
  • fainting
  • postural hypotension
  • worsening of original angina symptoms
  • failure of blood circulation
  • transient hypoxemia (deprived oxygen supply)

COMMON

  • headache
  • vertigo (spinning sensation) or dizziness
  • nausea
  • facial flushing
  • weakness

RARE

  • heart rhythm change

HOW TO MANAGE SIDE EFFECTS

Headache:  

Apply hot or cold-water bag on your head. Take rest in a quiet and dark room. Drink tea or coffee.

Vertigo or Dizziness:

Try to rest and relax. Avoid travelling, driving or standing for a long time. If symptoms worsen, consult your doctor immediately.

WARNING & PRECAUTIONS

PREGNANCY

GTN should be used with caution in pregnant women. Consult your doctor before taking this medicine.

BREASTFEEDING

GTN should be used with caution in breastfeeding women. Consult your doctor before taking this medicine.

DRIVING AND USING MACHINES

Do not drive or operate any machines if your ability is affected by this medicine.

ALCOHOL

Avoid consumption of alcohol while taking this medicine, because it might make you feel dizzier.

KIDNEY

GTN should be taken with caution in patients with severe kidney disease. Consult your doctor for advice.

LIVER

GTN should be taken with caution in patients with severe liver disease. Consult your doctor for advice.

ALLERGY

Do not take this medicine if you are allergic (hypersensitive) to glyceryl trinitrate.

LUNGS

Do not take this medicine, if you have severe toxic pulmonary oedema (swelling of the lungs). Consult your doctor for advice.

HEART DISEASE

Do not take this medicine, if you have severe heart problems like low blood pressure, cardiogenic shock, constrictive pericarditis, irregular heartbeat, circulatory collapse, pericardial tamponade (fluid around the heart) or acute myocardial infarction. Discuss the risks and benefits with your doctor.

OTHERS

Do not take this medicine, if you have:

  • severe anemia, G6PD deficiency, brain trauma or cerebral haemorrhage or increased intracranial pressure
  • an underactive thyroid gland or glaucoma

INTERACTIONS

Talk to your doctor, if you are taking,

  • medicines used to treat erectile dysfunction (e.g. sildenafil, tadalafil or vardenafil)
  • other medicine used to treat angina such as long acting nitrates (isosorbide dinitrate, isosorbide mononitrate)
  • medicines used to treat high blood pressure (propranolol, captopril)
  • medicines used to treat depression such as tricyclic antidepressants (e.g. amitriptyline)
  • medicines used to treat migraine (dihydroergotamine)
  • heparin (used to thin the blood)
  • riociguat (used to treat pulmonary hypertension)
  • diuretics (used to increase urine output)
