INTRODUCTION

ARBACE 5MG is a combination of Ramipril and Telmisartan which belongs to the group of medicines called Antihypertensive agents. It is used in adults to treat mild to moderate hypertension (high blood pressure). It also protects the function of kidneys particularly in patients who have both hypertension and type II diabetes mellitus.

Ramipril is an angiotensin-converting enzyme (ACE) inhibitor and telmisartan is an angiotensin-II receptor blocker. Together, they block the actions of certain enzymes (ACE) and hormones (angiotensin-II) in the body that causes narrowing of blood vessels and increase in blood pressure. As a result, they cause widening of blood vessels supplying the various vital organs (heart, brain, or kidneys) and makes the heart to pump blood more smoothly with less pressure.

Before taking ARBACE 5MG inform your doctor if you have diabetes or any kidney or heart problems. You must also inform your doctor if you are severely dehydrated or on a low-salt diet.

ARBACE 5MG is not recommended for use in patients with severe liver impairment or abnormally low blood pressure. It is also not suitable for use in pregnant or breastfeeding women as it may cause serious harm to the growing baby. Stop taking ARBACE 5MG and inform your doctor immediately if you become pregnant while undergoing therapy with ARBACE 5MG.

ARBACE 5MG may make you feel dizzy while standing up suddenly or changing positions, especially during the initial days of the therapy. Therefore, do not drive or operate any heavy tools or machines as a precaution. Also, avoid drinking alcohol as it might make you feel dizzier and drowsier.

ARBACE 5MG may alter your blood sugar levels and electrolyte levels. Therefore, your doctor will constantly monitor your blood sugar levels and kidney functions during the therapy to avoid the risk of unwanted side effects.

Other most common side effects of taking ARBACE 5MG are dizziness, headache, low blood sugar levels, tiredness, and weakness. Consult your doctor if any of these side effect worsens.

HOW ARBACE 5MG WORKS

ARBACE 5MG effectively reduces blood pressure and makes the heart to pump blood more smoothly. It contains two medicines namely ramipril and telmisartan. They both blocks the actions of certain enzymes (angiotensin-converting enzyme) and hormones (angiotensin-II) in the body that causes narrowing of blood vessels and increase in blood pressure. DIRECTIONS FOR USE

Always take ARBACE 5MG as directed by your physician. Swallow the medicine with a glass of water. Do not crush or chew the medicine. ARBACE 5MG can be taken with or without meals.

Your doctor will decide the correct dose and duration of therapy for you depending upon your age, body weight and health condition. Do not stop taking ARBACE 5MG without consulting your doctor even if you feel better during the mid-way of the therapy.

SIDE EFFECTS OF ARBACE 5MG

COMMON

  • dizziness, headache
  • low blood pressure or feeling dizzy while standing up suddenly or changing position

RARE

  • inflammation of the blood vessels
  • red swollen tongue
  • confusion or shakiness
  • loss of hearing or ringing sensation in ears
  • nail problems (separation or loosening of nail from its bed)
  • red, itchy, or watery eyes

Stop taking ARBACE 5MG and contact your doctor immediately if you experience any of the following side effects:

  • severe allergic reactions (such as skin rash, itching, swelling of the face, lips, mouth, or throat which can lead to difficulty in breathing or swallowing)
  • chest pain with faster heart rate, uneven or forceful heartbeat, chest tightness or more serious problems including heart attack and stroke
  • breathing difficulties or a cough
  • signs of blood or bone marrow problems (such as bruising very easily, bleeding for longer than normal, bleeding gums, purple spots under the skin, blotching on the skin or getting infections more easily than usual, fever, sore throat, feeling tired, faint, dizzy or having pale skin)
  • sign of pancreatitis (such as severe stomach pain which may radiate towards your back)
  • signs of liver problems (such as fever, chills, tiredness, loss of appetite, stomach pain, nausea, and yellowing of the skin or eyes)

HOW TO MANAGE SIDE EFFECTS

Nausea and Vomiting:

Take ARBACE 5MG with or just after a meal or a snack. Stick to simple meals. Avoid eating rich or spicy food. Consult your doctor if your nausea worsens.

Dizziness:

Try to rest and relax. Get enough sleep. Avoid driving or operating any tools or machines completely while you are feeling dizzy. Do not consume excessive alcohol, as it could aggravate your dizziness. Consult your doctor if your dizziness worsens.

Headache:  

Rest and relax. Keep yourself hydrated by drinking plenty of fluids such as water or electrolytes. Applying a pain-relieving balm on your forehead can be beneficial. Do not consume excessive alcohol, as it could aggravate your headache. Consult your doctor if your headache worsens.

WARNING & PRECAUTIONS

PREGNANCY

ARBACE 5MG is not recommended for use in pregnant women as it may cause serious harm to the growing foetus. Therefore, inform your doctor before taking ARBACE 5MG if you are pregnant, think you may be pregnant or planning to have a baby. If you become pregnant while undergoing therapy with ARBACE 5MG, stop taking it and consult your doctor immediately.

BREASTFEEDING

ARBACE 5MG is not recommended for use in breastfeeding women. Therefore, consult your doctor before taking it.

DRIVING AND USING MACHINES

Do not drive or handle any heavy tools or machines if you experience dizziness or drowsiness after taking ARBACE 5MG.

ALCOHOL

Avoid consumption of alcohol while taking ARBACE 5MG as it might make you feel dizzier and drowsier.

KIDNEY

ARBACE 5MG is not recommended for use in patients with severe kidney problems (such as renal artery stenosis). It should be used with caution in patients with other kidney problems (such as kidney transplantation). Therefore, consult your doctor before taking it.

LIVER

ARBACE 5MG is not recommended for use in patients with severe liver impairment (such as cholestasis or biliary obstructive disorders). It should be used with caution in patients with mild to moderate liver impairment. Therefore, consult your doctor before taking it.

ALLERGY

Do not take ARBACE 5MG if you are allergic to Ramipril or Telmisartan.

HEART DISEASE

ARBACE 5MG should be used with caution in patients with heart problems (such as heart failure, irregular heartbeat, coronary heart disease or problems related with heart muscle or valves). Therefore, consult your doctor before taking it.

OTHERS

ARBACE 5MG is not recommended for use if you:

  • have diabetes related kidney problems
  • have abnormally very low blood pressure

Before taking ARBACE 5MG, inform your doctor if you:

  • are severely dehydrated (due to diarrhea, vomiting or excess sweating)
  • are on a low-salt diet
  • have primary hyperaldosteronism (problems in adrenal gland)
  • are undergoing treatment for allergic reactions due to bee or wasp stings
  • have hyperkalemia (high blood potassium levels)
  • have collagen vascular disease such as scleroderma or lupus erythematosus
  • are undergoing any major surgery

Use in pediatrics:

ARBACE 5MG is generally not recommended for use in children and adolescents (aged below 18 years) due to lack of safety data. However, consult your doctor for advice before taking it.

Use in geriatrics:

ARBACE 5MG should be used with caution in elderly patients. Therefore, consult your doctor for advice before taking it.

INTERACTIONS

A. Drug-Drug Interaction:

Before taking ARBACE 5MG, inform your doctor if you are taking any of the following:

  • other medicines used to reduce elevated blood pressure (Ex. atenolol, propranolol, enalapril)
  • tricyclic antidepressants (used to treat depression) Ex. amitriptyline, nortriptyline, imipramine
  • antipsychotics (used to treat mental illness) Ex. chlorpromazine
  • baclofen (a medicine used for relaxing muscles)
  • amifostine (a kidney protectant used along with chemotherapeutic agents)
  • non-steroidal anti-inflammatory drugs (used for pain relief) such as indomethacin, celecoxib, ibuprofen, aspirin
  • sacubitril/valsartan (a combination used to treat chronic heart failure)
  • medicines used for cancer (Ex. temsirolimus)
  • medicines to prevent graft rejection (Ex. cyclosporin, tacrolimus, sirolimus)
  • diuretics (used to treat edema and hypertension) such as furosemide
  • potassium supplements or potassium-sparing diuretics such as spironolactone, triamterene or amiloride (used to retain or increase potassium levels)
  • trimethoprim alone or in combination with sulfamethoxazole (used to treat infections)
  • heparin (a medicine used for blood thinning)
  • corticosteroids (used to treat pain and inflammation) Ex. prednisolone
  • allopurinol (a medicine used to treat gout)
  • medicines used to treat diabetes (such as insulin, metformin, glimepiride, vildagliptin)
  • racecadotril (a medicine used to stop diarrhea)
  • aliskiren (a medicine used to reduce blood pressure)
  • lithium (a medicine used to treat various mental health problems)
  • anesthetics (used to create loss of sensations before surgery) Ex. proparacaine, lignocaine

Overdosage:

If you or anyone else accidentally took more ARBACE 5MG, consult your doctor immediately or visit the nearby hospital straight away. Symptoms of overdosage might include severe decrease in blood pressure and abnormal heart rate.
